In Mafia: The Old Country, you accompany Enzo Favara from simple “carusu” to the feared man of the family. This walkthrough guides you through all the missions from the prologue to the finale – with clear objectives, tactical recommendations, hints on hidden areas and tips for efficient 100% completion. We keep spoilers to a minimum where appropriate and highlight critical boss and key scenes with concrete suggestions for action.
How to get 100%: Basic principles & preparation
Chapter Replay unlocks after the first story sections and is your tool for catching up on what you’ve missed. Play main missions without pressure, mark hard-to-reach locations and return to them later. For collectibles, we recommend the following order: Newspapers → Photographs → Saints’ Cards → Mystery Foxes. You will find references at the end of each chapter in this guide.
Combat & Stealth: The game rewards a calm approach. Use cones of sight, listen for footsteps and use Enzo’s dodge roll defensively. For open battles, focus on short bursts, cover changes and controlled aggro. In boss phases, pattern reading, position changes and targeted finishers are crucial.
Build & equipment: Upgrade before challenging chapters. Find out which weapon shines in which scene and which mods really work in our Weapons & Materials Guide.

Walkthrough: Prologue to finale
Prologue – Tremori
Situation: Enzo’s everyday life collapses when a catastrophe shakes the mine. In narrow tunnels, you learn to dodge, parry and retreat in a controlled manner.
Objective: Stay calm, scout paths, keep cover. Confined spaces favor knives/short range; if enemies are grouped together, use noise to lure them into corners where only one at a time can reach them.
100% tip: Mark branches that you cannot complete under time pressure. You can work through them later in the replay and take photo spots/newspapers with you – see our overviews of newspapers and photographs.
Chapter 1 – Famiglia
Situation: Entering the family is a stealth test. You move through estates and outbuildings, observation comes before speed.
Strategy: Isolate instead of running. Wait for gaps in patrols, use visual interruptions (walls, visual cone edges). Practice counter-timing – a clean counter saves ammunition and often heals the momentum of the scene.
100% hint: There are often newspapers near the main paths in early gardens/side passages. Check small side doors and porches before the mission objective – and check off finds directly in the checklist.
Chapter 2 – Palio
Situation: Driving sequences and pursuits in a dense mix of lanes. The game is less forgiving of mistakes than a slow pace.
Strategy: Remember braking points, avoid collisions, drive “clean lines”. When in doubt, follow instead of overtaking until a safe takeover is possible. In the event of blockades: anticipate early, do not blindly push in peak lanes.
100% hint: Driving sections are rarely good farm time. Note collection points, but get them at your leisure via replay/exploration – e.g. photo spots on viewpoints(photography guide).
Chapter 3 – Pizzu
Situation: Dense district, several levels and lines of sight. Group patrols force you to be patient.
Strategy: “Segment” the path: Use house edges and passageways as safe islands. Use a silent approach to first eliminate alarm devices (whistle/signal). Change position after two kills to avoid chain alarms.
100% hint: Saints’ Cards are often found in backyards/side alleys. Do one last round before the mission objective – check the list: Saints’ Cards Guide.
Chapter 4 – Il Barone
Situation: Manor & outer areas with long lines of sight. Escort tasks require flank protection.
Strategy: First eliminate surveillance (archers, turrets), then clear paths. For escorts: slow down, don’t rush ahead, otherwise waves of flankers will spawn behind you.
100% hint: Several collection points are off the main paths (sheds, wine cellars). Keep the checklist open and scout briefly cross-country after each set piece(100% checklist).
Chapter 5 – Vendetta d’Onore
Situation: Dramatic story boost; change enemy types, vary ranges.
Strategy: Check equipment before starting: Main weapon + reliable secondary weapon. Do not hoard healing items – use them actively in phase changes. Never hold cover too long, otherwise pusher types will flank you.
100% hint: Some rooms have double entrances; collect at entrance A, exit via B to avoid respawn pulls. For suitable loadouts, see Weapons & Materials.
Chapter 6 – La Difesa Siciliana
Situation: You are defending bottlenecks over several waves.
Strategy: Play bottlenecks with “funnel” logic: Position barrels/doors, keep escape routes clear. Grenades for dense groups, pistol/knife for single targets. Always eliminate the first attacker early – he sets the pace.
100% tip: Take short collecting sprints between waves. Many foxes are surprisingly close to hotspots – if you see statuettes, mark them directly and tick them off later in the Mystery Foxes Guide.
Chapter 7 – Spirito Sportivo
Situation: Race/competition passages with tight tolerances.
Strategy: Better “clean” lines than full throttle. Inside-outside-inside through bends, do not cut edges if there are obstacles afterwards. Use checkpoints to set the rhythm.
100% hint: Collectibles are rarer in the race – make a note of them, then collect them at your leisure via exploration/replay (see photographs and newspapers).
Chapter 8 – Disgrazia
Situation: The situation is tipping, resources are scarce.
Strategy: Scout → Separate → Switch off. First understand layout, then separate targets individually. No “hero rushes”. Use noise sources to draw alarms into dead ends.
100% hint: Saints’ Cards are often found in side chapels/shrines – check niches before the final trigger(Saints’ Cards).
Chapter 9 – Industria
Situation: Factories & factory premises, machine noise drowns out footsteps.
Strategy: Use noise windows to get past groups. Verticality (stairs, walkways) provides an overview. Change position after each alarm so that opponents do not “run in”.
100% hint: Check photo spots of galleries/bridges before you leave the area – photography guide.
Chapter 10 – La Festa
Situation: Hidden action between many civilians.
Strategy: Inconspicuousness is everything: move in groups, combine cover with “natural” distractors (music, conversations). First key figures, then the rest. Cones of vision often overlap – patience.
100% hint: Decorative walls, backdrops and “behind the stage” often hide collector’s points. Turn the last round before the target marker (newspapers & photos).
Chapter 11 – Mattanza
Situation: One of the toughest battle sequences, several waves.
Strategy: Take max health + top weapons with you. Grenades for phase changes, keep cover “fresh” (never too long in one place). Focus targets first (shotcaller, shooter, then pusher).
100% hint: There are short safe windows between waves – quickly loot, check crates, continue. For suitable loadouts: weapons & materials.
Chapter 12 – La Forza del Destino
Situation: Multiphase boss battles demand precision.
Strategy: Read patterns, use short damage windows, never get greedy. Knife for staggers, gun for safe finishers. Change location after each phase to reset new adds/mechanics.
100% hint: Before the last phase, quick check of the collection levels; what is missing, you get via replay/exploration. Overview: 100% checklist.
Chapter 13 – Capiregime
Situation: Deep in the power structures, guards are more attentive, paths are convoluted.
Strategy: Pre-plan routes, observe cones of sight, break alarm chains (first observer, then runner). Silent kills speed up the entire sequence.
100%-Hint: Keep an eye on shrines/side chapels for Saints’ Cards: Saints’ Cards Guide.
Chapter 14 – La Merica
Situation: Final setpieces, chases, last battles.
Strategy: Check resources before starting the mission: Healing items, ammunition, best weapon setup. Anticipate pursuits (barriers, bottlenecks), keep escape routes open. Use the patterns you have learned – the game will check everything you have learned up to this point.
100% hint: If collectibles are missing, finish the chapter and use Replay/Exploration; stick to our compact routes in: Newspapers, Photographs, Saints’ Cards and Mystery Foxes.
